The Jasper Clinic is housed within Horizon Health’s Jasper Street Annex. Many visiting specialist providers at Horizon Health see patients at this clinic location. Note that the following visiting providers are not employed by Horizon Health. Phone communication, appointment scheduling, and needs regarding health records process in the same manner as the past—patients must contact their home offices directly.
